US Life Insurance Company administers Paragon�s Life Insurance Plan which begins automatically upon employment with Paragon.

Paragon provides Life Insurance to its employees free of charge. All full time employees are eligible and the basic coverage is:

• 1 (x) employee�s base annual salary
• 1 (x) employee�s base annual salary for accidental death or dismemberment
