Health & Dental Insurance

Paragon's medical plan is administered by UnitedHealthcare, one of the nation's premier health insurance carriers. With UnitedHealthcare, the selection of a primary physician is not required, nor does the need to obtain a referral before visiting a specialist.

Prescription Drug

UnitedHealthcare offers a three-tier co-pay plan for a 30 day prescription and 90 day supplies obtained through a retail pharmacy or a mail order program.

$10 - generic prescription
$15 - brand name prescription on United's "Preferred Drug" list
$25 - brand name prescription not on United's "Preferred Drug" list

The "Preferred Drug List" is not restrictive in prescribing or dispensing medicine, and considers therapeutic advantage or limitation relative to similar medicines available and potential side effects.

Vision Care

In addition to coverage for an eye exam every 24 months through the medical plan, you can obtain discounts on eyeglass frames and lenses through both the United medical plan AND the Aetna Dental plans. In fact, the Aetna Vision Discount Program, called Vision One, even enables members to save on contact lenses (10-20%) and Lasik surgery (15%).

One of the many advantages of the UnitedHealthcare Medical Program is www.myuhc.com. This secure, personalized site provides comprehensive information and instant answers to health plan questions.

Dental Insurance

Paragon's dental plan is administered by Aetna. Employees may choose between a traditional dental program� and a dental maintainance organization program (DMO).

The traditional plan permits the participant to select�a participating or non-participating�dentist.�This plan�requires a a claim form for reimbursement, and�has an annual deductible and maximum benefit.

The DMO, requires that a dentist be chosen from the Aetna national DMO network of participating dentists. This program covers most preventative care at no charge. There is no deductible or maximum benefit (except for orthodontia) and does not require the completion of a claim form. Additionally, every family member can choose a different DMO dentist.
